Job Description RN (Hospice Nurse) Job Description

Unity Hospice of Chicagoland is looking for a hospice field nurse.

  • to conduct initial assessments and admissions, skilled nursing visits, and provide case management services
  • travel is necessary on a daily basis, but travel between patient visits in minimized with geographic assignments

    Job Requirements RN (Hospice Nurse) What are the requirements for this position

    • Valid IL Registered Nursing License
    • Licensed driver with an insured vehicle
    • Case Management Experience in Hospice Preferred
    • Critical thinking, analytical, interpersonal skills a must
    • Compassion and care for others

    Compensation

    • $56,000 - $72,000 per year
