The Member Health Care Coordinator serves the members at Reach For Your Potential in regards to their medical needs. This position is responsible for scheduling and attending medical appointments for a small cohort of members with specialized medical needs. The Member Health Care Coordinator is also responsible for maintaining ADC medical records and completing quarterly reviews in compliance with the Department of Inspections and Appeals’ regulations. Communication with Reach For Your Potential staff, medical professionals, parents, guardians, and other agencies is essential in creating the best health care plan for each of the members. ESSENTIAL JOB FUNCTIONS:
Perform appropriate physical assessments, evaluations, and screening procedures for members.
Assess physical complaints of members and develop and implement a plan to address the complaint.
Manage medical correspondence for a small cohort of members with specialized medical needs, and attend appointments for these members in serious situations.
Provide direct care to members as needed with regards to medical needs.
Ensure compliance with doctors’ orders.
Ensure compliance with health-related provisions of the Iowa Administrative Code, CARF, Medicaid Waiver, and the Department of Inspections and Appeals.
Exchange appropriate information with community health care professionals.
Ensure the safe administration of medication.
Assist with monthly med. changeover.
Maintain the medical section of member ADC files.
Attend member service plan meetings and lead discussion on medical needs and health status with regards to service needs.
Assist in writing care plans/service plans for day programming.
Assist ADC Program Director to ensure compliance with health related provisions of the Iowa Administrative Code.
Teach medication administration course, and insulin course as needed.
Assist with medication and insulin observations every 6 month for all direct care staff.
Manage staff education in health related subjects including but not limited to medication administration, diabetic interventions, infection control, safety precautions, and medical protocols upon hire, annually, and as needed.
Complete other duties as assigned.
EDUCATION/ EXPERIENCE: RN certification from an accredited program, college, or university, and at least one year of post-degree experience in the delivery, planning, coordination, or administration of health or human services. SKILLS: Must be able to read, write, and communicate effectively with members and staff. Patience in working with adults with disabilities is necessary. Needs to have a caring attitude and be supportive and encouraging to members. A driving record that qualifies you to drive agency vehicles is required; sufficient automobile insurance to qualify you to transport members in your personal vehicle is preferred. SUPERVISORY RESPONSIBILITIES: Supervision includes: Member Health Care team, including: Member Health Care Assistant III, Member Health Care Assistant II, and three Member Health Care Assistant Is. General supervision of medically related issues is exercised over Direct Care staff and Facilitators. EQUIPMENT TO BE USED: Medical equipment within the typical realm of a nursing certification will be used. Training for specialized adaptive equipment for individual members will be provided. Must be able to operate computer and other office machines such as fax, calculator, telephone, etc. TYPICAL PHYSICAL DEMANDS: It is preferred that the employee is able to lift at least fifty pounds. Must be willing and able to assist with toileting and other activities of daily living as required. Requires sitting, standing, bending, and reaching. Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, telephone, and other office equipment. WORKING CONDITIONS: Typically works in an office setting and in the community while accompanying members to medical appointments. Transportation of members is a large part of this position. Occasionally called upon to work overtime or odd schedules. This is an hourly position with any hours worked over 40 in a week is paid at the overtime rate.
