Job Description

This position requires traveling to patient's homes.

Summary:   This position provides professional nursing care to patients in their home.  Delivers care to patients utilizing the nursing process of: assessment, planning, intervention, implementation and evaluation.  This position must effectively interact with patient, significant others, physicians, and other health team members while maintaining standards of professional nursing.  The RN must provide direct supervision of LPN’s and Aides assigned to patient case load.  The documentation must demonstrate working knowledge of policies and procedures as well as federal, state and local regulations that impact the practice of nursing in the Hospice Setting.

a.       Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.

b.      Confidentiality of all data, including patient, employee and operations data.

c.       Quality Assurance and compliance with all regulatory requirements.

d.      Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior.

 

3.    Supports and participates in common teamwork:

a.       Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.

b.      Uses tactful, appropriate communications in sensitive and emotional situations.

c.       Follows up with appropriate management, personnel regarding reported complaints, problems and concerns.

d.      Promotes positive public relations with patients, family members and guests.

e.       Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.        

                      

Essential Functions:

  1. Assess the patient; collect and document the patient’s current health status and medical history.  Complete and/or review admission data as defined by policy and procedure.  Make accurate and ongoing assessment of patient’s status and respond appropriately.
  2. Plan/Implement; set priorities for nursing action in logical sequence according to patient’s needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards.
  3. Develop and revise the Comprehensive Plan of Care based on all aspects of the initial and on going assessment working in cooperation with other members of the IDG.  The CPOC should include physiological, psychosocial, and environmental factors.
  4. Based on ongoing assessment of patient status document patient care which reflects nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations and progress toward goals.  Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted.
  5. Communicate with the physician/medical director regarding the patient needs, the nursing assessment and recommendations for care as needed.  Accurately transcribe physician orders.  Documentation of order is handled promptly as required by policy and procedure.
  6. Administer medications, IV’s and treatments according to policy and procedure.  Observe, report and document the patient’s response to above.
  7. Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician’s orders and policy and procedure.
  8. Demonstrate ability to handle emergency situations in a prompt, precise and professional manner.
  9. Demonstrate ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.  Organize job functions and work area to be able to effectively complete varied assignments within established time frames.
  10. Demonstrate working knowledge of policies and procedures and ability to reference them from appropriate sources.
  11. Give a thorough, concise report to staff on patient, which communicates pertinent information in a timely manner.
  12. Demonstrates working knowledge and implementation of infection control practices.
  13. Develops aide plan of care, based on the assessment of the patient personal care needs.  Revises and update the aide plan of care as needed or required by policy and procedure.
  14. Supervises the provision of services provided by the aide and provide feedback on performance as indicated.  Assist in annual evaluation of the aide performance.
  15. Supervises the provision of services provided by the LPN based on the plan of care and provides feedback on performance as indicated.  Assist in annual evaluation of the LPN performance.

Non-Essential functions:

  1. Performs other duties as assigned.  Assignments are completed in a timely and accurate manner.

Working Conditions:  While performing the duties of this job, the employee is regularly required to talk, hear, sit use hand to finger, reach with hand and arms, and lift 60 pounds.

Job Requirements

Minimum Education:

Education: Minimum – Associate Diploma in Nursing with current Mississippi License.

Minimum Experience:  One year practice as RN, prefer previous acute care experience or home health/hospice experience.

Miscellaneous Requirements:  Valid Driver’s License, Auto Insurance, Reliable Transportation.
