Social workers provide services that include a comprehensive psycho social assessment, identification of problems and needs, supportive counseling, discharge planning, and assessment and facilitation relating to accessing resources. Additionally, they are responsible for a bereavement assessment of family members. Social workers are responsible for the development of a plan of care in conjunction with other interdisciplinary team members and regularly participate in the interdisciplinary team meetings. They actively support and promote the hospice’s vision, mission and overall philosophy.
Bachelors or Master’s degree in Social Work approved by the Council on Social Work Education. A Master’s degree is preferred.
Minimum of one (1) year supervised experience in the healthcare field or during a supervised internship.
A current license to practice as a Social Worker in the State of Nevada.
Must be a licensed driver and possess an automobile that is insured in accordance with state and/or organizational requirements and is in good working order. Must maintain a clean driving record.
Hospice and Palliative Care Certification is preferred.
