Summary: The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. Administers skilled nursing care to patients requiring intermittent professional nursing services. Teaches and supervises the family, and other members of the nursing team. Each staff nurse performs nursing services for residents within a specific area.

Essential Duties and Responsibilities:

• Completes assessments of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Monitoring through observations: patient, staff and physician feedback; written and verbal communication; quality improvement/quality assurance activities; records management.

• Regularly re-evaluates patient nursing needs.

• Initiates the plan of care and makes necessary revisions as patient status and needs change.

• Uses health assessment data to determine nursing diagnosis.

• Develops a plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

• Counsels the patient and family in meeting nursing and related needs.

• Provides health care instructions to the patient as appropriate per assessment and plan of care.

• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

• Performs as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

• As directed by Clinical Supervisor, while in Case Manager role, is responsible for other Registered Nurses, Licensed Practical Nurses, and Home Health Aides who may be working on a particular home care case for which the primary RN is responsible for managing.

• Responsible to communicating with all disciplines involved in the patient's care (i.e., Physical Therapy, Occupational Therapy, Speech Therapy, etc.)

• Initiates and applies preventive and rehabilitative care techniques.

• Uses a systematic approach to plan, assess, implement, and evaluate consistency between the plan of care and the care plan.

• Communicates changes in the patient's condition to the team leader, physician and the other team members.

• Participates in on-call duties as defined by the on-call policy.

• Ensures that arrangements for equipment, medication and other necessary items and services are available. Uses equipment and supplies effectively and efficiently.

• Performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.

Licenses/Certifications:

• Current licensure in state and CPR certification.

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

• Active/current agency health requirements.

• Must complete required department competencies to validate and maintain competency.

Education/Experience:

Education: Graduate of an accredited school of nursing with Associates or Bachelor's degree.

Experience: Minimum one (1) year of clinical experience in hospital setting – medical/surgical; with two (2) years community/home health or skilled nursing experience preferred. Management experience not required; but responsible for supervising home health aides.
