The Hospice Aide is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Hospice Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Program, between the Program and other Company departments, and between the Program and all external stakeholders. The Hospice Aide will provide personal care, supportive services, education, and light household duties in a plan of care developed by the RNCM. The aide will communicate changes to the Registered Nurse Case Manager and complete documentation to reflect assigned duties set by the Registered Nurse Case Manager. The Hospice Aide will utilize the IDT team to assist in this process. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.

Education and/or Experience
High school diploma or general education degree; or one to three months related experience and/or training; or equivalent combination of education and experience.
Ability to successfully complete competency evaluation program. This includes a written and a skills evaluation. Ability to work a flexible schedule, to include some evenings and weekends. High integrity, including maintenance of confidential information such as patient records. Must be able to travel.
Certifications, Licenses and Registrations
Certified Nursing Assistant certification required.
Connecticut- Certified Nursing Assistant designation by the CT board of nursing.
Maine - Certified Nursing Assistant designation by the ME board of nursing.
Massachusetts - Certified Nursing Assistant designation by the MA board of nursing.
New Hampshire – LNA certification by NH board of nursing.
Valid driver’s license and auto liability insurance.
Other Skills and Abilities
Must be able to tend the geriatric and the terminally ill.
