POSITION SUMMARY: The Staff Social Worker provides social casework services to clients to augment other services of the branch. A primary focus is the evaluation of social functioning, particularly as it relates to medical problems, and initiation and follow-up of referrals to other resources as applicable. All care delivered is to follow Standards of Practice for the particular state and must be in compliance with company policy and regulatory requirements.

POSITION RESPONSIBILITIES:

1. Performs social work assessments per company policies and procedures. Identifies social problems and evaluates their severity and interrelatedness to the medical situation.

2. Identifies strengths, weaknesses, resources, and support systems of the patient and family. Identifies the patient's needs for additional services and makes community referrals as appropriate.

3. Assesses the family financial situation, including the financial impact of the illness, as appropriate.

4. Notifies the primary nurse/therapist or Clinical Manager/Branch Director of significant problems/needs and findings.

5. Delivers quality care through appropriate and skillful interventions and observations.

6. Completes initial assessment and ongoing notes to reflect assessment and ongoing progress toward expected outcomes. Provides updates to the patient's Plan of Care.

7. Documents referrals to community resources and the follow-up of those referrals.

8. Maintains excellent assessment skills and clinical knowledge and expertise.

9. Assumes responsibility for ongoing interdisciplinary assessment of patient.

10. Develops and evaluates the Plan of Care in partnership with patient, representative, and caregiver.

11. Completes visit notes thoroughly with emphasis on documenting skilled services provided and home bound status documentation. Submits notes and Daily Visit Records on timely basis in conjunction with branch policy.

12. Understands criteria for coverage and reimbursement for home care. Secures necessary information from patients to verify appropriate payor source and ensure continued coverage as assigned.

13. Provides information about community resources, self-help groups, and support groups to patient and caregivers as appropriate.

14. Arranges for and conducts case conferences with other groups, agencies, staff as needed in conjunction with the primary nurse/therapist or Clinical Manager/CFSS.

15. Assists team members and the physician to understand significant social and/or emotional factors as they relate to health problems.

16. Maintains established productivity standards on a regular basis.

17. Assists in orientation for new employees as assigned.

18. Exhibits strong documentation skills and assures that client documentation is in compliant with branch and regulatory policy.

19. Participates in data collection for Quality Improvement by reporting complaints, making pertinent suggestions to improve efficiency, customer service, customer satisfaction and/or cost containment. Participates in the QAPI program.

20. Follows organizational policies and procedures regarding infection control, disposal of hazardous waste and use of protective devices as evidenced by documentation and the absence of related incident reports.

21. Supports the principles of the Corporate Compliance Program, attend orientation and in-service updates.

22. All other duties as assigned.




QUALIFICATIONS:

1. Must hold a Master's Degree in Social Work.

-For MSW – minimum of six (6) months experience required.

2. Must be certified in state(s) of practice.

3. Home care experience preferred.

4. Must have exceptional customer service skills and be able to communicate well with referral sources, patients, family members, and branch employees.

5. Must have/maintain a valid driver's license and auto liability insurance.

6. Must be flexible with work schedule.
