Summary: Responsible for managing all aspects of maintaining and protecting organization clinical records. Maintenance and retention of home health and hospice patient medical records consistent with State and Federal regulations, agency policy and procedures, and CHAP standards. Backup support for other clerical workers as needed. Responsibilities: Ensures clinical record systems are maintained in compliance with state, federal and CHAP regulations.
Maintains comprehensive working knowledge of state, federal, and CHAP documentation regulations and serves as a resource for appropriate organization personnel.
Protects all clinical records through the establishment and implementation of control procedures for all open and closed records.
Protects the confidentiality of patient and organization information through effective controls and direct supervision of clinical records.
Forwards copies of clinical records to authorized users according to policy.
Completes an administrative record audit following patient discharge and forwards the record to the Clinical Supervisor for clinical audit.
Monitors aged unsigned physician orders and submits second and third requests for signed orders to overdue physicians, calls or visits the offices of seriously overdue physicians.
Supervises the use of the clinical records information system and maintains a comprehensive working knowledge of the system including upgrades and enhancements.
Plan, prioritize, organize and complete work to meet established objectives.
Should be good steward of all agency financial resources, including but not limited to arrangements for equipment and other necessary items and services needed.
Performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.
Assists on a variety of special projects and other duties as needed.
Follows agency policies and procedures. Maintains confidentiality of patients and agency information.
Completes and submits all documentation in a timely manner according to department policy.
Attends in-service and education as required.
Adheres to Federal/ State guidelines rules regulations, statutes and law.
Follows the Code of Conduct policy.
Complies with corporate and departmental policies and procedures.
Valid driver's license and auto insurance;
Must complete required departmental competencies to validate and maintain competency.
Education: High School Diploma or equivalent certification required
Experience: Minimum of one (1) to two (2) years' experience in Healthcare Office Administration or Medical Field; Medical Records background/experience with home health and/or hospice experience preferred. Homecare Homebase experience highly preferred.
