Summary: Provision of nursing services in accordance with applicable state licensure status, and in compliance with agency policies and procedures, applicable health care standards, and governmental laws and regulations. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. These services are performed in accordance with the physicians' orders and under the direction and supervision of an RN, Clinical Supervisor or Agency Administrator.
Essential Duties and Responsibilities:
Administers skilled nursing care with appropriate documentation, under the supervision of a registered nurse and the physician directed plan of care. Assists RN or physician in performing specialized procedures and duties. The LPN/LVN performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.
Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Assists the registered nurse or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Clinical Supervisor.
Uses equipment and supplies effectively and efficiently.
Prepares and submits documentation in a timely manner according to agency policy.
Performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.
Follows agency policies and procedures. Maintains confidentiality of patients and agency information.
Participates in appropriate team conferences, staff meetings, in-service and committees with physicians and agency staff to ensure continuity of care.
Completes and submits all documentation, including clinical and progress notes in a timely manner according to agency policy.
Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
Possesses and maintains CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Active/current agency health requirements.
Must complete required department competencies to validate and maintain competency.
Education: Graduate of an accredited practical nurse or vocational nursing program.
Experience: Minimum of one (1) year clinical nursing experience in hospital setting medical/surgical; with community/home health or skilled nursing facility experience is preferred.
