Specific Duties: Case Manager RN 1. Transitions of Care 2. Provide patient education/care planning with patients telephonically 3. Create care plan and assessments in computer database 4. Work in a call center environment with metric measures daily 5 . Team environment; service as patient advocate for hospital/LTAC discharge for disease conditions. 6. Work closely with Interdisciplinary Team. Must have qualifications/experience: 1. Bilingual 2. Minimum 2 years of care management experience 3. Active Texas RN licensure; Florida licensure additionally is preferred 4. High customer service and telephonic skills needed to actively engage patients telephonically 5. Computer Skills (Word, Excel). Able to navigate through different programs.
