Job Description

Clinical Coordinator (Home Healthcare)

The Clinical Coordinator of Private Duty nursing assists the Location Director in managing the clinical integrity of the in-home nursing program.
 

Responsibilities:

  1. Employee will complete admission and recertification documentation and will ensure that orders for changes in the nursing plan of treatment in response to patient’s condition are submitted in a timely manner.
  2. Employee will act as a resource to staff and families identifying and accessing information on unfamiliar disease processes treatments.
  3. Employee will assist field nurse to develop creative ways to instruct patients, families, and other patient representatives in the implementation of the plan of care and other concepts and activities necessary to promote the patients’ health, safety and independent living.
  4. Employee completes all necessary documentation accurately, timely and legibly according to agency and regulatory requirements.
  5. Employee monitors patient charts to ensure the accurate and complete documentation of clinical progress notes, which reflects observations, interventions, and evaluations pertaining to patient, care management and services provided, in accordance with home health care specifications.
  6. Employee will conduct and participate in multidisciplinary case conferences as needed.
  7. Will assess level knowledge and educate families and caregivers about safety in the home and accessing emergency support in collaboration with the Clinical Coordinator.
  8. The employee will maintain an objective and consumer oriented viewpoint in dealing with patients, families, and visitors.
  9. The employee will project a positive agency image, serving as a liaison, to referral sources, discharge planners, insurance case managers, and patient care personnel.
  10. The employee will collaborate with Clinical Management Team to develop in-service training programs for staff.
  11. The employee will participate in PI activities specific to home care.
  12. The employee will receive training the in the computerized staffing program and will enter clinical information as requested.


 

Resource Nurse / Home Healthcare / Registered Nurse (RN)

 
 

Job Requirements  
  1. Current license as a registered nurse in the state.
  2. No disciplinary board action related to nursing license.
  3. Demonstrates  knowledge of home ventilators, apnea monitors, oxygen, trachs and trach care, suctioning, special feedings, G-tubes, N/G tubes, J-tubes and IVs. 
  4. Annual Basic Life Support Certification.
  5. Evidence of negative TB skin test of X-ray.
  6. Satisfactory criminal records check.
  7. Two years experience in PICU, NICU or other related pediatric healthcare environment.
  8. Knowledge of quality care for medically complex children.
