Job Description Judge Healthcare is actively seeking an RN Case Manager for a well known healthcare company in the Birmingham, AL area!
Primary Responsibilities: 
  • Provide high touch Case Management services to members 
  • Provide high-touch outreach to PEEHIP members while in the hospital and post-discharge.
  • Work on-site at the assigned hospitals with highest volume, communicating with members providers. and hospital discharge planners to ensure members know and understand their care plan including next level of care (e.g., home, SNF, AIR). 
  • Contact/document attempted outreach with all  members discharged from Acute Inpatient Rehabilitation (AIR) facilities while they are in the hospital.
  • Contact high risk members with discharge plan to go to SNF to support high touch services during hospital discharge planning to prevent readmission to hospital. 
  • Provide smooth handoff to community based Community Transitions RNs or  Case Management RNs for follow up when home from an inpatient stay 
  • Provide patient education to assist with self-management 
  • Educate members on disease processes

  • Required Qualifications: 
  • Current, unrestricted RN license in the State of Alabama 
  • 5+ years’ experience as an RN in a clinical setting
  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment •Internet access
  •  Preferred Qualifications: 
  • Bachelor's Degree 
  • A background in community care or home care 
  • Case Management experience 
  • Certified Case Manager (CCM) 
  • Experience with discharge planning or transitional care services 
  • Strong organizational skills and multitasking abilities will be keys to success
