Job Description

RHD Passages Montrose is a residential program for adults with serious mental illness. Residents participate in services that focus on building skills of community living. The goal of the program is to maximize the ability of each individual to live as independently as possible.  Passages Montrose is seeking Bachelors-level ADL Counselors.

Job Responsibilities:

  • Performs tasks as part of a multidisciplinary treatment team aimed at working towards the needs and desires of the consumer
  • Collaborates with Resident Manager and treatment team in providing trauma-informed, best practice care for all consumers.
  • Meets individually on a regular basis with consumers to discuss strengths, needs, abilities, preferences.
  • Works with consumers to create person-centered treatment goals and objectives
  • Works with consumers in the achievement of treatment goals and objectives
  • Works with consumers to build skills of community living; including ADL skills, coping skills, educational skills, vocational skills
  • Facilitates psychoeducational/creative arts groups
  • Provides case management/advocacy services for consumers in community
  • Communicates with nurse regarding medical issues and concerns
  • Collaborates with other members of treatment team to problem solve issues for all consumers
  • Completes regular clinical documentation that reflects the course of treatment as required by funding/licensing/accreditation  
  • Performs tasks in support of the physical and mental well-being of the consumers
  • Provides consumers with supervision, support and assistance with all activities of daily living
  • Works collaboratively with CNA ADL staff and RN to assure that health care needs are addressed
  • Performs tasks to assure that program is in compliance with the policies and regulations of RHD, LTCRP, DSAMH, AND CARF
  • Renews on time and provides documentation of all necessary trainings certifications and licenses
  • Provides services in a manner consistent with the Rights of Clients as outlined by Delaware Code, Section 1121, entitled Patient’s Rights
  • Completes all documentation as required 
  • Communicates with on call staff regarding urgent programmatic issues
  • Participates in maintaining the cleanliness and order of the physical property
  • Displays a positive attitude toward the job and tasks of the job
  • Participates in supervision with the Resident Manager/Unit Director for the purpose of ongoing professional development
  • Performs tasks in a manner that upholds the Values of RHD
  • Interacts with consumers in a manner that reflects compassion and understanding.
  • Interacts with peers, supervisors and the community in a manner that reflects a respectful, mature and professional outlook
  • Interacts with consumers in a manner that is not confrontational or argumentative
  • Interacts with consumers in a manner that is respectful of the consumer’s ability to make his or her own decisions, while also managing the responsibility of the program
  • Other duties as assigned
  • Minimum Qualifications:

  • Bachelor’s degree in a related field
  • 2 years of previous experience in social services
  • Valid Driver’s License
