Job Description

Position Title: Central & Northeast Regional Director of Case Management – South Dakota

Reports to: State Director of South Dakota

General responsibilities include, but are not limited to: Provide administrative oversight for the company’s short and long term goals within designated regions by ensuring that staffing, financial, compliance, customer service and more importantly the goals and needs of those served are met.

Essential Functions and Responsibilities:

  1. Follow all policies and procedures set forth by the company and South Dakota Department of Human Services, Division of Developmental Disabilities and Home and Community Based Services (HCBS) rules.

  2. Provide case management duties with designated cases and when staffing levels or consumer needs are not being met.

  3. Hires, trains, manages, develops and coaches staff to maximize performance and outcomes.

  4. Establishes staffing levels and monitors caseloads to maximize performance and financial resources.

  5. Establishes and maintains positive relationships with individual, families, guardians, state officials and team members.

  6. Establish and monitor quality outcomes for consumers and the region.

  7. Ensures that all case records, plans and other related documents are properly maintained.

  8. Coordinate meetings with individual, families and guardians, and members of the interdisciplinary team as required.

  9. Records work and billing time in accordance with company policies.

  10. Tracks and monitors specified data points.

  11. Take necessary action to maximize revenue and limit expenses.

  12. Participate in short and long term planning process to ensure continued quality and financial success.

  13. Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.

  14. Performs evaluations and assessments to meet the needs of individuals served.

  15. Assists individuals served with applying for financial assistance, residential planning, vocational, recreational and educational desires, healthcare, in home supports, day services, legal, nutrition, transportation, social, and other related services and resources.

  16. Advocate for services that will support the individual’s success.

  17. Maintains all case records.

  18. Prepares, updates and monitors person centered plans including utilization of discovery and Person Centered Thinking (PCT) Tools.

  19. Provide support to individuals so they can participate in and direct the person centered plan development process.

  20. Other duties as assigned.

  21. Comply with all standards to assure the health and safety of all individuals.

  22. Report to work as scheduled.

Qualified candidates must possess a Bachelor’s degree in Human Services, Social Work, Sociology or Psychology, a minimum of 3 years working in a supervisory or management role, at least 2 years of Case Management experience and have a valid Driver’s license.  Certification as an Accredited Case Manager (ACM) or Certified Case Manager is preferred.  The selected candidate will be expected to travel throughout each region(s) served and demonstrate excellent communication skills.
