POSITION SUMMARY:

The Physical Therapist Asst. – Home Health is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Program, between the Program and other Company departments, and between the Program and all external stakeholders. The Physical Therapist Asst. – Home Health is responsible for providing medically prescribed, goal oriented therapy in accordance with the Physician Plan of Care, company policies and professional Standards of Practice.PRINCIPAL ACCOUNTABILITIES

  • Implements treatment goals, plans and programs identified by the Physical Therapist.
  • Documents the care and submits paperwork in an accurate and timely manner to update and maintain the client medical record
  • Under the direction of the Physical Therapist, provides therapeutic treatments and evaluation of equipment needs as applicable.
  • Assists the Physical Therapist to plan for discharge as appropriate.
  • May participate in agency quality improvement programs.
  • Ensures the growth and profitability of the company through the responsible use of company resources and educating the community to our services.
  • Participates in regularly scheduled interdisciplinary team meeting to coordinate the care of the client & family, exchange information & problem solve & receive staff support and education.
  • Applies the policies & procedures of the agency and the rules and regulations of State and Federal regulatory agencies and other certified agencies in providing care
  • Under the direction of the PT, provide skilled therapeutic interventions designed to meet the patient’s need for: functional skills training, pain reduction, strengthening exercises, balance and coordination exercises, posture training, prosthetic training, cardiovascular rehab, instruct on home safety and instruct the patient/family on exercise programs
  • Is able to utilize moist hear, portable whirlpool, paraffin baths, ultrasound, electrical muscle stimulator, goniometer, assistive device for gait training, gait belt and any other commonly used devices or equipment in home setting.
  • Accepts other assignments deemed appropriate.
  • Participates in agency quality improvement programs when requested.
  • Adheres to established company policies.

  • Strong oral and verbal and interpersonal communications are needed.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Education and/or Experience

    Currently licensed in state of practice.  Minimum one year experience in a home health setting.

    Other Qualifications

    Ability to successfully complete competency evaluation program.  This includes a written and a skills evaluation. Ability to work a flexible schedule, to include some evenings and weekends. High integrity, including maintenance of confidential information such as patient records.  Must be able to travel.

    Certifications, Licenses and Registrations

    Valid driver’s license and auto liability insurance. Valid Physical Therapist Assistant License in state of practice.

    Other Skills and Abilities

    Must be able to tend the geriatric and the terminally ill.
