Job Description RN Patient Assessment Coordinator

TITLE: RN Patient Assessment Coordinator

 

JOB OVERVIEW: This position will coordinate efforts with others to provide support and expertise for patients with urgent/emergent conditions and healthcare advice following best practices and clinical practice guidelines. This position will also work collaboratively with clinical care teams in the coordination of care for certain patient symptoms and conditions.

AREA OF

ASSIGNMENT: Clinic Network

HOURS: Monday – Friday, 7:30 am – 4:30 pm or as necessary to meet departmental needs. 

RESPONSIBLE

TO: Director, Patient Resource Center  

Prerequisites:

  1. RN required. License must be current to practice in the State of Washington.

  2. Bachelor’s degree preferred.

  3. Minimum three years’ experience as a registered nurse in an outpatient clinic setting, preferred.

  4. Telephone triage experience highly preferred.

  5. Demonstrated clinical skills and clinical expertise with an emphasis on patient triage.

  6. Demonstrated skills for problem solving & decision making.

  7. Understands patient safety requirements, quality improvement models, and the transitional interfaces between clinic, hospital and community.

  1. Demonstrated excellent interpersonal, oral and written communication skills with diverse patients, employees, leaders and other organizational and community representatives.

  2. Demonstrated flexibility and ability to prioritize and manage multiple demands.

  3. Able to work collaboratively with a wide range of disciplines in the health care team in order to develop a patient’s plan of care.

  4. Demonstrated knowledge and use of the nursing process to identify physical, psychosocial, and developmental needs and interventions to promote favorable outcomes. Demonstrates a commitment to patient centered care.

  5. Possesses the knowledge related to the use of various computer and health care related concepts.

  6. Able to accept responsibility, work independently and take initiative.

  2. Unique Job Functions:

    1. Responsible for calls from patients and authorized individuals requiring or requesting triage assessment and will utilize the approved triage guidelines to offer the appropriate level of care to ensure an optimal outcome.

    2. Conduct comprehensive assessments utilizing evidence based guidelines, along with insight of the nursing process, and a proven ability to critically analyze information from the patient and the EMR in the decision making process.

    3. Provides homecare, advice and/or education to callers that respects the cultural, spiritual, intellectual/educational, and psychosocial differences of individuals and preserves caller’s autonomy, dignity and rights.

    4. Maintains confidentiality, HIPAA and PHI compliance. 

    5. Facilitate communication with providers and other healthcare organizations as appropriate based on the clinical situation to facilitate safe transitions of care.

    6. Utilize translation services as necessary to obtain the clinical information required to complete an assessment.

    7. Assist with the evaluation, assessment and plan of care for patient portal messages, faxes and other methods of communication via the Patient Resource Center (PRC) as needed.

    8. Access patient information via EMR; educates patients utilizing principles of active listening; communicates based on patient’s level of understanding and comprehension and has knowledge of the teaching principles of health care information to pediatric, adult and geriatric populations.

    9. Document telephone encounters and decision making in the EMR in a concise and accurate manner, utilizing the appropriate templates and the patient’s own words as appropriate.

    10. Provides leadership support and guidance to call center staff for clinical issues and situations.

    11. Participates and contributes to the interdisciplinary team process.

    12. Collaborates across the organization as needed to assist with quality improvement initiatives.

    13. Collaborates with operational leadership and the professional training specialist to add input for ongoing competency assessment processes and tools needed.

    14. Maintains awareness and adheres to the National Patient Safety Goals and Clinic Network Patient Safety initiatives as pertinent to job role.

    15. Reports concerns about patient safety utilizing the error reporting system and reports to the director as needed.

    16. Assists with the investigative process for specific patient safety reports as directed by the director.

    17. Will participate in process improvement related to patient safety as directed by the director or clinic leadership.

    18. Will participate in the development, review and revision of Network policies and procedures as they relate to quality and safety improvement initiatives based on job role.

    19. Participates in assigned meetings to gain in-depth understanding of issues relevant to the quality and safety of patient care.

    20. Consistently demonstrates the ability to assess a situation from a variety of perspectives, consider several alternatives, and choose the appropriate course of action for the situation.

    21. Acts as a clinical expert for consultation questions related to telephone triage.

    22. Makes recommendations for initial or ongoing staff development related to education or competency assessment.
