Utilizing a collaborative process, Case Managers will performthe Case Management activities of assessing, planning, implementing,coordinating, monitoring and evaluating options and services required to meetan individual’s health needs. The Case Manager will use communication andavailable resources to promote quality, cost-effective outcomes.
The CaseManager helps identify appropriate providers and facilities throughout thecontinuum of services while ensuring available resources are used in a timely andcost-effective manner in order to obtain optimum value for both the client andthe reimbursement source. While performing the Case Managementactivities, the Case Manager will involve and demonstrate knowledge of thefollowing Care Components of Case Management: processes and relationships, health care management,community resources and support, service delivery and psychosocial.
Current and valid license to practice as a Registered Nurse (ADN or BSN) in the state of Idaho; Bachelor’s Degree is preferred.
One to three years of acute clinical experience. 3-5 years is preferred.
Excellent interpersonal communication and negotiation skills.
Strong analytical, data management and PC skills.
Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
Ability to work independently and exercise sound judgment in interactions with physicians, payors, patients and their families.
Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.
Understanding of pre-acute and post-acute venues of care and post-acute community resources.
Current AHA CPR for Healthcare Providers certification
Professional certification as a Case Manager is preferred.
