Job Description As a member of the multidisciplinary care team, the LPN provides general nursing care to residents. The LPN contributes to

overall resident care which is guided by the resident care plan, facility policies and procedures, and professional standards of

practice. The LPN directly supervises the Resident Assistants.

Essential Job Functions

Clinical record reflects accurate and timely documentation to ensure continuity and quality of

resident care.

Functions as the primary contact person on assigned shift. Effectively supervises resident

assistants to ensure compliance with plan of care.

Prepares, administers and charts routine and emergency medication and treatment as

ordered. Demonstrates knowledge of medications and their side effects. Demonstrates

knowledge, skills and ability in IV Therapy to include initiation, administration and


Accurately receives and follows through on new orders from physicians and consults.

Accurately receives and follows through on lab slips.

Completes required quality control measures, i.e., narcotic counts, glucometer and unit

specific activities, as evidenced by documentation, observation and feedback.

Organizes workload to complete responsibilities within a designated time period as

evidenced by observation and feedback.

Obtains blood specimens accurately and quickly, using proper technique, with attention to

patient identification, patient care, safety and infection control practices.

Follows, updates and maintains the residents care plan as that it accurately reflects the

residents current status to include but not excluded to medication changes, dietary changes,

care requirements, skin/wound care updates, and recent falls.

Assess resident for possible and current skin issues with each scheduled skin assessment.

Implement any prevention measures needed to ensure residents skin integrity is

maintained. Provide appropriate wound care per physician order and policy as necessary.

Notify resident and appropriate family members on all resident care/condition changes to

include but not limited to weight changes, medication changes, radiology/lab results, and

onset of new conditions.
