Through a continuum of services, Woodward Community Based Services, a division of Sequel Youth and Family Services, provides education, counseling and supervision to individuals and families in their home community. Our professionals work in conjunction with local resources to develop a strength-based plan to support success for our consumers. Services include:
Woodward Community Based Services is hiring a Part Time Behavioral Health Intervention Specialist for Winterset, Iowa and surrounding areas. The job duties include:
Provide Skill Development services to clients’ age 4-twenty one years of age and their families in the home, school or community as outlined in the Individualized Treatment Plan
Fulfill expectations of LPHA order.
Develop and maintain a working relationship with the community and associated agencies.
Complete service documentation (service notes, reports and releases, etc.) within required timeframes.
Maintain client notepad by documenting all communication or attempts to communicate with referring worker and parents or lapses in services great than one week.
Coordinate with other service providers involved with the individual including, but not limited to the individual’s mental health provider, psychiatrist, health care provider, school/educational staff, child welfare services providers, and juvenile court service providers.
Attend and participate in all required trainings.
Maintain proper norms and security of youth and provide crisis intervention as necessary.
Attend and participate in team meetings weekly (full time) or twice monthly (part time).
Comply with Code of Conduct, Compliancy, Confidentiality and Core Value requirements.
Uphold agency communication standards.
Follow Iowa Code Chapter 24.3(4.11) code of ethics which states: staff will not engage in uninvited solicitation of potential clients, BHIS will not be provided in the home of Intervention Specialist, BHIS will not be provided at the convenience of Intervention Specialist, and that staff not have a conflict of interest and/or dual relationship with the individual receiving services. This includes, but is not limited to, child care provider, coach, juvenile court school liaison, family member, or personal friend of the individual. Must be at least 3 years older than the individual served who is less than 1eight.
Maintain valid driver’s license and sufficient liability insurance coverage on the vehicle used for transportation of clients.
Abide by The Health Insurance Portability and Accountability Act of 1996 (42 USC & 201 et seq.)
All other duties as assigned.
