Job Description

Responsible for providing aide services according to the resident home health service agreement, while maintaining the resident’s dignity and level of independence. Responsible for the safety of our residents. Conducts business according to the Senior Star Mission, Vision, Values and Goals.

PART-TIME Monday-Friday Evening Shifts, 20-30 hours a week. $10/hour

Essential Job Functions:
Responsible for the Assistance of Residents (as determined by the Service Agreement & Plan of Care)

  • Provides care according to the plan of care and within the guidelines of laws, regulations, and Senior Star policies and procedures
  • Provides hands-on assistance with activities of daily living (mobility, bathing, dressing, grooming, etc.) to residents.
  • Provides light tidying of resident apartments while performing duties related to service agreement within a resident apartment.
  • Provides resident safety checks
  • Escorts residents to meals and activities (residents may be ambulatory or use a mobility device)
  • Provides oxygen assistance as appropriate
  • Properly documents care provided to residents
  • Reminds residents to take their medication at prescribed times as assigned
  • Measures resident blood pressure and heart rate and follows instructions related to holding medications according to blood pressure and heart rate parameters and immediately notifies Home Health Administrator.
  • Completes documentation accurately in accordance with medication assistance procedures
  • Informs the Home Health Administrator immediately of reported and/or observed changes in resident condition or care needs, including abnormal vital signs.
  • Application and removal of therapeutic compression stockings as ordered by medical provider and per the plan of care.
  • Will adhere to isolation precautions when necessary
  • Assists in care plan development for the residents
  • Responds to calls for assistance in a timely manner
  • Assist residents in preparing to leave the community for appointments, as directed by the Home Health Administrator.
  • Encourages residents to be as independent as possible
  • Accomplishes duties patiently and to the best of his or her ability, regardless of a resident’s cognitive ability.
  • Keeps work areas clean at all times
  • Computer data entry, filing, and office tasks as needed, as directed by the Home Health Administrator.
  • Follows established procedures to maintain resident safety and prevent injury to self and/or others
  • Attends staff meetings and in-services.
  • Completes twelve mandatory computer training modules annually and submits documentation to the Home Health Administrator.
  • Completes annual competency testing (written & skills demonstration)
