We are hiring for a Telephonic Case Manager with an elite managed care organization. This is an exciting opportunity for you to focus on member needs by assessing, monitoring and optimizing patient health across the continuum of care.

The Telephonic Care Coordinator will manage and track all members who require a comprehensive approach to immediate and ongoing care of their complicated and/or catastrophic illnesses with duties such as care plans, progress notes, assessments, correspondence, referrals, authorizations and more:

  • Provides and coordinates community resources and referrals
  • Provides member education on disease process
  • Collaborates with the Interdisciplinary Team to incorporate best practices, assess outcomes and develop individualized care plans
  • Identifies members, through the daily inpatient census, who are a readmit within the last thirty days and/or have specific disease conditions
  • Assesses member medical, social, environmental and functional needs and addresses identified needs with the members medical team to develop a comprehensive treatment plan
  • Educates members and providers on services available to address chronic disease, chronic illness management or other identified reasons for utilization of the ER
  • Reviews prior authorization/service requests with Medical Director for determination
  • Assists members with provider referrals, appointments and transportation, as needed
  • Minimum Qualifications:

  • Registered Nurse or Licensed Practical Nurse in the state of Montana
  • 5+ years strong clinical skills and judgement
  • Previous clinical or Case Management experience
  • Ability to navigate multiple computer systems and strong typing skills
  • Superior critical thinking & problem solving skills
